Physical Exam - Each participant must have a yearly physical prior to his/her participation in sports. This portion of the Form MUST be signed and dated by a physician each year (May 23 or after) indicating that the student is physically able to participate. Physical Form
ASB Card - All athletes are encouraged to purchase an ASB card. Proceeds from ASB Cards pay for referees, dues, etc. Cards can be purchased at the Finance Office or by clicking here.
Fairshare Donation - The "Fairshare" is a fundraising program designed to support MVHS athletic teams with equipment and uniform needs. In addition, each athletic team is scheduled on a rotation to purchase new uniforms. In most cases the rotation is a 4-year cycle. Towards the end of each sports season, and after an accounting forecast for new uniforms are calculated, any remaining "fairshare" funds will be transferred to the individual sport teams for that season with a maximum transfer of $1000.00 per team. Donations can be made at the finance office or by clicking here. This donation is not required for participation in the athletic program.
Transportation Fee - All athletes must travel to and from athletic contests in school-provided transportation. The Transportation Fee is a $100.00 per sport. If a student cannot pay this fee, he/she may obtain a waiver from the Athletic Director. Fees can be paid at the finance office or through the Athletic Clearance at the time of registration.
